Do I have to complete the biometric screening/Physician Results Form to participate in the program?

No. One of the greatest things about the Employee Wellness Program is that it is not all or nothing. Eligible employees may participate in any or all components. The program is designed to improve overall employee health, physical condition, and mental wellness through preventative health screenings, medical exams, and physical activity.

However, to receive (or renew) a free combination pass to the Kettering Recreation Center, you must complete the annual biometric screening/Physician Results Form.
