Rumpke Qualified Disabled Resident Rate- Affidavit
To be eligible for Rumpke’s Qualified Disabled Resident Rate, participants must do the following:
- Provide a signed note from their physician explaining they are physically unable to take their trash to the curb.
- Sign an affidavit stating neither they nor anyone else in the household are physically unable to take their trash to the curb.
- Signed physician’s note and affidavits can be faxed to 937-296-2550 or brought or mailed to:
