The Combined Health District of Montgomery County offers the following immunizations: DtaP, Polio, Hib, Hepatitis B, MMR, Prevnar, Rotateq, Td, Tdap, and influenza seasonally.  Chicken Pox vaccine is available by reservation only.  Menactra, Gardasil, and Hepatitis A are available on a limited basis to those that qualify.  All children ages birth-18 years welcome. 

There is a $15 office visit fee and a $10 per shot administration fee.  All fees for state supplied vaccines are based on ability to pay.  No child will be turned away based on guardian ability to pay.  No money is collected at the site, fees will be billed.  Please bring your child’s shot record and insurance card to every visit.
