The program runs for 12 months, from November 1 to October 31. Each year the program resets the totals and starts over as of November 1st.
There are two types of points you can earn: 1 – Activity (exercise) points and 2 – Non-Activity (Lecture / screenings / education) points. The active points requirements is 420 points. The non-active points requirement is 80 points.
Active points are awarded based on the type and duration of exercise. There is a 6 points per day maximum for activity points. The point break down for active points is found below:
Non-Active points are awarded for health screenings, preventative wellness checks and wellness lectures. The points vary by screening and type of activity. There is a 20 points per day maximum for non-activity points. The non-active points are found directly on the submission form below.
As you exercise, submit your Activity points in one of the following ways:
1 – Submit your points on-line via the City’s Website. On Human Resource Dept page > click “Employee Wellness” link On Employee Wellness page > click “Wellness Forms” link On Wellness Forms page > click “Online Participation Form” (save to favorites) direct link: http://www.ketteringoh.org/wellness-points/
2 – Submit a hard copy of your points to the ‘Parks, Recreation and Cultural Arts – Administration office’ by printing out the Participation Logs found on the website. Use the Wellness Program Point System to find the number of points for your exercise type and duration. Remember to keep a copy of your logs for your records.
3 – Scanned or electronic copies can be sent to WellnessPts@ketteringoh.org. Forms sent to other addresses will be returned unprocessed. Forms other than the unaltered online form(s) will also be returned unprocessed. Remember to keep a copy of your logs for your records. Non-Activity and Group Exercise Points:
Submit your Non-Activity points using the “Wellness Non-Activity Point Form (to be used for Non-Activity points and Group Exercise Classes only). ” Complete this form by placing an “X” in the box by the appropriate activity and obtain signature (initials or physician’s office stamp are acceptable) as instructed on the form. DVD Lecture viewings can be signed by your department representative. Total your non-activity points and enter the total in the appropriate box. Please note that points from blood pressure screenings will only be accepted monthly and points from blood glucose screenings will only be accepted quarterly.
Submit your Group Exercise Class points using the “Wellness Non-Activity Point Form (to be used for Non-Activity points and Group Exercise Classes only) .” You may use this form for your Group Exercise classes or if you need more room, use the additional Group Activity Form available on the Website. Have the instructor sign the Group Activity Form. Instructors at the Kettering Facilities (KRC and KFWC) will have a supply of the Group Exercise forms on hand. Simply ask them for a form at the end of the class. Attach all of your Group Activity Forms to the “Wellness NonActivity Point Form (to be used for Non-Activity points and Group Exercise Classes only) ” and place the total Group Exercise points in the appropriate box. Send the hard copy of the form, along with any Group Exercise Class forms to ‘Parks, Recreation and Cultural Arts – Administration office.’ Remember to keep a copy for your records.
You can submit your points daily, weekly, monthly or quarterly.
IMPORTANT: Points must be submitted before the quarterly deadlines.
